Information we collect
When you apply
- Name, date of birth, email address and contact phone number
- Residential address, mailing address and whether you live on Tribal lands
- Your qualifying program, or household size and income if you qualify by income
- Device and combo preferences, switching details (current carrier) and, if you bring your own phone, its IMEI
- Your certifications and consent choices
Our online application does not collect Social Security numbers. The official Lifeline verification process may separately ask for the last four digits of your SSN or a Tribal ID number to verify your identity.
When you contact us
Your name, email address and the contents of your message.
When you visit the website
Standard server logs (such as IP address, browser type and pages requested) used for security and troubleshooting. This site loads fonts from Google Fonts, which receives your IP address when fonts load. We don't use advertising cookies or third-party analytics on this website.
How we use information
- To verify your eligibility for Lifeline and California LifeLine
- To enroll you, activate service and transfer your number
- To prepare and ship your devices
- To provide customer support and send service notices
- To comply with program rules, audits and legal obligations
- To send marketing emails, only if you opt in (you can unsubscribe at any time)
How we share information
We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We share information only as needed to provide your service:
- Program administrators and regulators: USAC and the National Verifier, the California LifeLine Administrator, the FCC and state commissions, as required by Lifeline rules.
- Service providers: our network provider, shipping carriers, and email and technology vendors, under contracts that limit their use of your information.
- Legal requirements: when required by law, subpoena or to protect rights and safety.
Retention
We keep information as long as needed to provide service and as required by Lifeline program record-keeping rules and applicable law, then securely delete it.
Security
This website uses HTTPS encryption. Access to applicant information is limited to staff who need it. No method of transmission or storage is 100% secure. Please don't email full Social Security numbers or passwords to us.
Your choices and rights
You can ask to access, correct or delete your personal information, subject to program record-keeping requirements. Email privacy@combowireless.com.
California residents
If you're a California resident, you have rights under the California Consumer Privacy Act, as amended, where it applies: to know what personal information we collect and how we use and share it; to delete it; to correct it; to opt out of its sale or sharing (we don't sell or share it); to limit the use of sensitive personal information; and not to be discriminated against for exercising these rights. Submit requests to privacy@combowireless.com. We'll verify your identity before responding.
Children
Applicants must be at least 18. We don't knowingly collect information from children through this website.

We'll update this page when our practices change and revise the effective date above.
